Note: By filling out this form doesn’t guaranty you to become a member of IHCC unless approved by IHCC working group. This form is to be submitted via email. Upon submission, your application will go through a formal process for consideration by the IHCC working group meeting. Once your application is processed, you will be advised of the outcome. If approved, your details will be entered into members register and made available for public as per IHCC constitution. If you do not wish your particulars or any part of your details to appear in the members register, please indicate in this form.
